Adult Foster Care (AFC), also called Adult Family Care, is a program designed to help people on MassHealth age 16 or older with disabilities who cannot live alone safely. This program provides 24-hour family living for elderly persons and disabled adults who due to frailty, chronic illness or advanced age who prefers to live in a family setting rather than in a nursing home or other facility.
AFC adults live with trained paid caregivers who provide daily care. Caregivers may be family members(except legally responsible relatives), or non-family members.
